Why does my neck stay tight no matter what I do?

Persistent muscle tightness is usually a symptom, not the problem. Guarding is a response, not a diagnosis

When a spinal joint loses normal motion, the muscles around it tighten to limit what that joint can do. That is protective in the short term and counterproductive over months. Massage, stretching and heat relieve the guarding temporarily; if nothing changes at the joint, it comes back — which is exactly the pattern most people describe by the time they call us.

The clinically useful question is not "how do I relax this muscle." It is "what is this muscle protecting, and why."

Tension-type headache and the upper cervical spine

Headache that starts at the base of the skull and wraps forward, is worse late in the day, and is worse after desk or driving hours is frequently cervicogenic — originating in the neck rather than the head. It is one of the most common presentations we see, and one of the most commonly mistreated, because the pain is felt somewhere other than where it is generated.

Examination of upper cervical motion, joint loading and the surrounding musculature usually sorts this out quickly.

What evaluation looks like here

Evaluation for muscle guarding and tension-type headache at Champion Chiropractic & Spinal Injury includes:

Range of motion: Segmental and global range of motion, measured rather than estimated.

Orthopedic and neurological screening.

Palpation for the specific segments driving the guarding pattern.

Radiographs when indicated, including motion views where instability is suspected, with a plain-language explanation of what we found and what it means.

Where imaging is warranted, the emphasis is on objective and demonstrative spinal injury imaging and reports — findings that are measured and reproducible, supported where appropriate by computerized radiographic mensuration analysis.

The old injury nobody connects

A meaningful share of chronic tightness traces back to a specific event — a crash, a fall, a sports injury — that happened long enough ago that the patient no longer links the two. Symptoms faded, the mechanical problem did not resolve, and the tissue adapted around it.

Part of the initial evaluation is a history detailed enough to surface that event, and imaging when the findings suggest a structural reason the tissue is behaving this way.

Frequently Asked Questions

Can a chiropractor help with tension headaches? When headache is driven by upper cervical joint dysfunction — cervicogenic headache — chiropractic evaluation and treatment are directly relevant. When it is not, examination should identify that and redirect you appropriately.

Why does the tightness always come back after a massage? Because massage addresses the muscle and not the joint the muscle is protecting. Relief that lasts days rather than weeks usually points to an unaddressed mechanical cause.

Could this be from a car accident years ago? It can be. Soft tissue and ligamentous injury can remain symptomatic long after the acute episode resolves. The history and imaging will tell us.

Do I need X-rays? Not automatically. Imaging is ordered when examination findings indicate it, and we will explain why before it is taken.

What are your hours? Monday through Thursday, 8:00 to 10:00 AM and 3:00 to 6:00 PM. Friday and Saturday by appointment. Closed Sunday.
