How to Find Your Car Insurance Dec Page After a Car Accident

Your "dec page" (declarations page) is a short summary of your car insurance coverage. After a car accident, it shows whether you have Medical Payments (Med Pay) coverage and how much. We use it to check your benefits for you, before your first visit.

You can usually get it in 2 minutes from your insurance app or website.

Get your dec page in 3 steps

  1. Open your insurance app or website. Log in.
  2. Find "Documents." Sometimes it says "Policy" or "ID Cards & Documents." The dec page may be inside a file called "Renewal" or "Policy notice."
  3. Open "Declarations" or "Dec page." Take a photo or screenshot, or download it. Then send it to us (see below).

Can't log in or can't find it?

Call the phone number on your insurance card. Say: "Please email me my declarations page." Your insurance agent can also send it. Or check the papers or email your insurance company sent when you renewed.

Send it to us

Easiest way: text a photo or PDF of your dec page to (515) 777-1014. Put your name in the message. It goes straight to our team.

Or upload it here:

If police came, or you exchanged insurance information with the other driver, send a photo of that form or report too. If you don't have it yet, that's okay.

What we look for on your dec page

You do not need to understand it. We read it for you. Here is what we check:

  • Medical Payments (Med Pay): coverage that can help pay medical bills after a crash. Not every policy has it. We check the amount.
  • Policy dates: your policy must have been active on the day of your accident.
  • The vehicle: the car on the policy should match the car in your accident.
  • Uninsured / Underinsured Motorist (UM/UIM): coverage that can matter if the other driver has no insurance or too little.

Your insurance ID card is not enough. It usually does not show your coverage amounts.

Open your car (auto) policy. Home and renters policies also say “Medical Payments.” That is not the same. We need the policy for the car you were in.

Common questions

What is a dec page?

“Dec page” is short for declarations page. It is a short summary of your car insurance policy. It lists who is covered, which vehicle is covered, the dates of the policy, and how much coverage you have.

Is the dec page the same as my insurance card?

No. Your ID card shows you have insurance. The dec page shows what your insurance covers and how much. We need the dec page.

Why does a chiropractor need my dec page?

After a car accident, some policies include Medical Payments (Med Pay) coverage that can help with medical bills. The dec page shows if you have it and how much. We check this before your visit so you know what to expect.

What if I can’t find it before my visit?

That’s okay. Bring your insurance card and your phone. Our team will help you find it.

What if the car belongs to someone else?

Send us the dec page for the policy on the car you were in. If you were a passenger or in a family member’s car, tell us. We will help you work out which policy to check.

Does insurance always pay for my care?

It depends on your policy and your claim. We check your coverage before your visit and tell you what we find. We bill the insurance company directly when your coverage allows it.
